    How to determine if the quality improvement initiative is successful
    Careful assessment is an important part of evidencebased practice Initial assessments of depressive symptoms can help determine possible treatment options and periodic assessment throughout care can guide treatment and gauge progress

    Mental health care provided in most practice settings uses procedures that are different from treatments considered effective in research settings Quality improvement interventions can effectively increase the rate of care for evidencebased depression Unfortunately a managed behavioral health care environment is unlikely to participate in largescale quality improvement interventions in psychotherapy because there are no effective manageable measures to prove that the psychotherapy procedures provided are evidencebased In this study we present preliminary findings on patientreported measurement methods regarding the extent to which the therapist uses cognitive behavioral therapy (CBT) interpersonal therapy (IPT) or psychodynamic therapy

    Psychotherapy for depression is a suitable area to begin to develop measures for patient reporting in psychotherapy procedures The high prevalence of depression and the early age of onset make depression the leading cause of disability worldwide Two clear forms of psychotherapy for depression CBT and IPT have been clearly established and can be used as effective interventions for depression However many patients with depression do not receive evidencebased psychotherapy Efforts to improve quality have greatly improved the prognosis of depression but without a method of measuring quality care it is not feasible for psychotherapy Effectively measuring the degree of use of psychotherapy procedures consistent with CBT or IPT can enable behavioral health management organizations to document the impact of quality improvement interventions on psychotherapy for depression

    The current method of determining whether psychotherapy is evidencebased is not feasible for quality improvement work Trained observers encode audio or videotapes of the meeting to determine adherence to specific psychotherapy Although this is not a means of psychotherapy in randomized trials and routine care settings caregivers report compliance with MST principles has proven to be a reliable predictor of prognosis for children

    To assess the effectiveness of our initiatives we should encourage clinicians to assess the state of the patients (with their consents)

    Certain methods of assessing the result of treatments and initiatives are different checklists inventories systematic assessment programs and rating scales for patients The most effective one adequate one in this setting I believe is MontgomeryÅsberg Depression Rating Scale (MADRS)

    The 10item MontgomeryÅsberg Depression Rating Scale (MADRS) measures severity of depression in individuals 18 years and older Each item is rated on a 7point scale The scale is an adaptation of the Hamilton Depression Rating Scale and has a greater sensitivity to change over time The scale can be completed in 20 to 30 minutes Patients can finish this rating scale to show if their mental health is getting better However if the patients are finishing these scales in different environments or without proper instruction they might lean towards certain answers and provide false result To eliminate the possibility of this problem clinicians should make sure that the patients’ answers are coherent throughout the procedure of treatment
